London Homeopathy Privacy Policy

About this document

This document will help you understand how I deal with your personal information, that is information that could identify, or is related to the identity of an individual.


What personal information do I collect?


In order to treat you I will ask you to provide certain information. This includes:

Name,

Gender

Home address,

Date of birth and nationality

Contact details, including telephone numbers and email address

Health details and medical history

To this, over time I will add details of the conditions for which you have consulted me and the remedies and other therapies that I have prescribed or recommended.


How do I collect this personal information?


I collect and update personal information from you directly. This is usually at the point of your initial consultation. The information is collected via a “medical history form” which I ask you to complete prior to the initial consultation. At the point at which you provide your personal information, I will also request that you provide consent for me to store and use your data. Your consent is required in order to ensure my compliance with data protection legislation.


How do I use your information?


I use the information to analyse the condition for which you have consulted me and to prescribe remedies and other therapies.

I will communicate with you by email, text messaging or other digital methods, by telephone and by post.


With whom do I share your data?


I do not share your information with third parties.


How do I store this information?


Any personal information gathered during consultation sessions is securely stored in hard copy in a locked cabinet in my home office. Only I have access to those paper-based files.

I make sure that the computer used for emailing patients is password protected, and that the computer is securely stored when not in use. My computer is screen locked or logged out of when I am away from my desk.

Care is taken to prevent virus attacks by ensuring my computer has virus protection software and undergoes regular software updates.

Care is taken when opening email attachments and when visiting new websites.

The personal data kept electronically consists of your email address and phone number, along with any messages exchanged.

London Homeopathy may occasionally send you an email reminding about the next appointment date or outlining your prescription (remedies to be taken and when to take them only).

Your mobile number and any text messages we have exchanged are stored on my iPhone and is backed up electronically on the iCloud to protect against accidental loss.

Individual emails sent to individuals are held securely on the servers of my email providers, currently Gmail and Hotmail.


How long do I keep your information?


I need to keep your information for as long as you consult me. Since patients often return for more consultation after a period of absence, I will keep your information for seven years after your last consultation. At that point your file will be securely destroyed and any digital information will be erased from my computer systems.


How your information can be corrected?


To ensure that I have correct and up to date information, you need to inform me of any changes you believe I should make to the personal information I hold. You can do this by contacting me or by any other methods previously described.


Your rights


Under GDPR you have the following rights:

To obtain copies of the personal information that I hold about you. You can make a request to do so my contacting me and I will endeavour to respond within 14 working days

To require me to correct the personal information I hold about you if it is incorrect

To require me to erase your personal information – this must be a written request, with your reasons for requesting erasure clearly stated

Please note that these rights may be limited by Data Protection legislation, Contract law, Criminal law and Human Rights legislation, and I may be required to refuse requests where exemptions apply.


A data breach


In the event of a data breach occurring, I would immediately inform everyone affected, and take whatever steps necessary to minimise impact. I would also report the breach to the ICO.
